| (1) The goods listed in the Catalogue of Import Commodities subject to Quality Licensing System must obtain a quality license issued by the state inspection and quarantine department and attach a "safety mark" before applying for inspection and quarantine. The goods in the Catalogue of Compulsory Certification shall be certified and marked with THE CCC mark. (2) The following entry goods shall be subject to the examination and approval of the State Inspection and quarantine organ before being submitted for inspection: Animals and plants, animal and plant products and other quarantine objects from the epidemic area; B. Quarantine objects that are subject to special permission of the State; C. Animals and animal products that are not processed, used or sold within the jurisdiction of the inspection and quarantine body of the port of entry after entry, or that are transported to the inspection and quarantine body of the port of destination for further quarantine control only after on-site quarantine and disinfection of the outer package by the animal and plant quarantine body of the port of entry; D. Products of imported pigs, etc. (3) The entry goods that have been inspected before shipment should still be inspected in accordance with relevant regulations upon arrival at the port, and the inspection structure of the inspection and quarantine body of the port shall be the final result. Claims for compensation for goods that fail to pass the inspection and quarantine shall be handled in accordance with regulations. Instructions on entry cargo declaration form (items must be complete, accurate and clear.) (6) Total value of goods: according to the total value of goods listed in the contract, invoice or customs declaration form. (7) Mark and number: to be filled in according to the mark number listed in the contract, foreign invoice or delivery order. (8) Country (region) of departure: means the country (region) from which the means of transport carrying the goods starts to depart. Port of departure: refers to the name of the port of departure of the means of transport carrying the goods. Port of stop: refers to the name of the port of stop before the goods arrive at the destination after shipment. (11) Port of entry: refers to the name of the port at which the means of transport carrying the goods first stops at the port of entry. (12) Port of destination (place) : means the port of delivery (place) at which the goods are expected to arrive last. (13) Date of arrival: fill in the actual date of arrival of the incoming goods at the port of destination. (14) Date of completion of unloading: fill in the actual date of completion of unloading of inbound goods. (15) Trade country (region) : fill in the country or region from which the incoming goods are purchased, and fill in according to the contract. B/L/Waybill No. : fill in the attached B/L or Waybill No., if there is a second way B/L, fill in both. (17) Validity of claim: The claim period shall be filled in according to the contract, and the deadline shall be specified. (18) License/Approval Number: refers to the import Safety quality License number or the import Animal and Plant Quarantine License number of the goods that are subject to official approval. (19) Container specifications, quantity and number: fill in the container specifications of this batch of goods and the full names of corresponding quantity and container number respectively. If there are more containers, additional pages can be added. (20) Storage place of goods: refers to the specific storage place of this batch of goods. (21) Special terms and other requirements concluded in the contract: refer to the quality and health terms concluded by both parties in the trade contract and the special requirements of the inspection and quarantine of the goods. (22) Use: refers to the use of this batch of goods, such as planting, eating, milk, viewing or performing arts, companion, experiment, medicinal use, feeding, other, etc. (23) Attached documents: tick "√" in the corresponding "□" of the documents actually provided to the inspection and quarantine institution. If not listed, fill in the blanks. (24) Foreign-invested property: according to the actual situation, the foreign-invested property shall be marked with the "□" √. (25) The contents of other columns in the inspection declaration form shall be filled in or selected according to the facts. |
